Voucher Drop Down Menu

    I am trying to add a voucher purchase menu to my site. I have used the one posted on the support page but can't figure out how to make it a drop down menu to choose different amounts. Can anyone help? Here is the code (without my personal account info)

    To sell a voucher you need to include four fields in a form posting into the new voucher script. The four fields are; the value of the voucher in a hidden form field along with the recipients name, email address and a message.

    <INPUT TYPE="HIDDEN" NAME="userid" VALUE="gg12345">

    <INPUT TYPE="HIDDEN" NAME="gift_amount" VALUE="15.00">**I want to have different amount options - how can I make this a drop down?**

    Name: <INPUT TYPE="TEXT" NAME="gift_name">
    Email: <INPUT TYPE="TEXT" NAME="gift_email">
    Message: <TEXTAREA name="gift_message"></TEXTAREA>

    You can have as many forms like this on a page and you can use an image for the submit button. You may choose to use a drop down list for the gift_amount so people can select different voucher values

    The standard dropdown list would be:

    <select name="gift_amount">
    <option>Choose the value</option>
    <option value="5.00">5.00</option>
    <option value="10.00">10.00</option>
    <option value="15.00">15.00</option>
    <option value="20.00">20.00</option>
